Accommodated Learning Request
- Please see Accessible Learning Centre's website for more information on the types of accommodations that are available.
- If you have a Family Doctor it is best to have this completed by them as they know you best.
Allergy Injection
- For ongoing allergy injection therapy
General Check up/ Complete/ Physical
- A comprehensive visit that exams your body by looking, feeling and listening.
Head Injury
- Recent head injury? Please contact the Health Centre directly as the clinic will be booking you with multiple Health Providers to ensure appropriate assessments can be done.
Illness
- Acute Illness (i.e., sore throat, flu, cough, etc.)
Initial Mental Health Request
- Never been seen by the UofL Health Centre for Mental Health? Book an Initial Mental Health appointment.
- If you have any documentation from another provider regarding your mental health please bring it in with you for your appointment.
- What can we do:
- Start Medication
- Continue Medication
- New Diagnosis
- Refer to Psychiatry or a Therapist
- Please note you will be sent some assessment forms via email the day before you appointment. Please complete these prior to arriving for your booked apt.
Mental Health Follow Up
- Continuing care for mental health concerns.

- Not Mental Health related, for new Mental Health medication requests please select a Mental Health Initial Appointment.
- Refill request
- If the Health Centre has never filled this prescription, please ensure you bring a prescription list with you to your appointment.
