The above information is collected, used and disclosed by authority of the Post-secondary Learning Act and in compliance with the Alberta Freedom of Information and Protection of Privacy Act. The consent given in these Release Forms will remain in affect until such time as you are no longer a student in the Faculty of Education.

Student Work Release Form

We would like to be able to post your work as future exemplars if applicable and selected. Your work will be anonymous when used as examples in class or in related research unless you would like it cited. Un-graded versions will be used in all cases.

To facilitate communication between class participants an email list serve including your email address will be posted to course web sites. Your cooperation in this endeavor is appreciated.

I give the Faculty of Education permission for my image, either in photograph or videotape form, to be used for Academic purposes. This includes, but is not limited to, publications and Faculty of Education web pages.
